New Establishment License Application – Complete All 3 Steps
- Visit the Environmental Health Tab below and select the establishment type you are applying for.
- Complete the relevant application and forms listed and submit it by mail or email to:

Once your application and payment have been received and processed, Environmental Health Specialist, Brooke will contact you to schedule your pre-inspection.
Blood Pressure Checks
Having high blood pressure puts people at risk for heart disease and stroke which are the leading causes of death in the United States. Measuring your blood pressure is the only way to know if you have high blood pressure! The Taylor County Health Department offers blood pressure checks in the office. Appointments are NOT required, but we recommend you call ahead to ensure a nurse will be available. We suggest a $5 donation for this service.
