New Establishment License Application – Complete All 3 Steps
- Visit the Environmental Health Tab below and select the establishment type you are applying for.
- Complete the relevant application and forms listed and submit it by mail or email to:

Once your application and payment have been received and processed, Environmental Health Specialist, Brooke will contact you to schedule your pre-inspection.
Ear Flushing

The Taylor County Health Department offers ear flushing for anyone ages 5 years and older. This service helps to remove excess earwax or other foreign objects in the ear canal. This service is not meant to take the place of a doctor visit or to provide recommendations for hearing-assistive devices.
