New Establishment License Application – Complete All 3 Steps
- Visit the Environmental Health Tab below and select the establishment type you are applying for.
- Complete the relevant application and forms listed and submit it by mail or email to:

Once your application and payment have been received and processed, Environmental Health Specialist, Brooke will contact you to schedule your pre-inspection.
Hearing Checks
The Taylor County Health Department partners with the schools in our county to provide hearing tests on a yearly basis, and we can also provide this service in our office. We utilize a digital hearing screener to help determine if a referral is needed based on the results of the screening.
