New Establishment License Application – Complete All 3 Steps
- Visit the Environmental Health Tab below and select the establishment type you are applying for.
- Complete the relevant application and forms listed and submit it by mail or email to:

Once your application and payment have been received and processed, Environmental Health Specialist, Brooke will contact you to schedule your pre-inspection.

The Taylor County Health Department offers professional toenail care services by appointment. Care is provided by a registered nurse and includes trimming and filing of toenails.
Appointments are available at the Health Department, at select senior dining sites on a regular schedule, or in your home for individuals who are homebound.
Please contact us to schedule an appointment or to learn more about available locations and dates.
Service Cost: $40 in office/senior dining site
$45 in home
In Home Mileage Fee: 1-10 miles $5
In Home Mileage Fee: 10+ miles $10
