New Establishment License Application – Complete All 3 Steps
- Visit the Environmental Health Tab below and select the establishment type you are applying for.
- Complete the relevant application and forms listed and submit it by mail or email to:

Once your application and payment have been received and processed, Environmental Health Specialist, Brooke will contact you to schedule your pre-inspection.

Please note that there may be times warming locations may be closed due to extreme weather. We recommend calling ahead or checking social media pages in advance to ensure the location will be open.
Ready.gov has many resources for different types of emergencies that may occur. Winter is inevitable in Wisconsin, so individuals should be prepared for a winter storm. Always keep a full tank of gas, have your cell phone charged, and keep an emergency supply kit in your vehicles!

- Flashlight
- Warm Clothes
- Blankets
- Bottled Water
- Non-Perishable Snacks
