New Establishment License Application – Complete All 3 Steps
- Visit the Environmental Health Tab below and select the establishment type you are applying for.
- Complete the relevant application and forms listed and submit it by mail or email to:

Once your application and payment have been received and processed, Environmental Health Specialist, Brooke will contact you to schedule your pre-inspection.

Tattoos & Body Art
The Taylor County Health Department is an Agent to the Wisconsin Department of Agriculture, Trade and Consumer Protection (DATCP). As an agent to DATCP, we manage the licensing and inspection program of all retail food, lodging and recreation facilities in Taylor County. Additional information for each type of facility can be found below. If you are starting a new business or adding a service to your current business (example selling food), it is required to call the Taylor County Health Department at 715-748-1410 to schedule a pre-inspection. Pre-inspections will assist you in preparing your establishment for licensing.
To apply for a new license:

- Fill them out completely.

Body Art Establishment is any premises where a tattooist applies a tattoo to another person, or premises where a body piercer performs body piercing, or both.
