New Establishment License Application – Complete All 3 Steps
- Visit the Environmental Health Tab below and select the establishment type you are applying for.
- Complete the relevant application and forms listed and submit it by mail or email to:

Once your application and payment have been received and processed, Environmental Health Specialist, Brooke will contact you to schedule your pre-inspection.
Vision Checks
The Taylor County Health Department partners with the schools in our county to provide vision checks on a yearly basis, and we can also provide this service in our office. We utilize a SPOT vision screener to help detect different vision issues in individuals as young as 6 months. The camera is a non-invasive way to get a reading and can help us provide a referral to an eye doctor. Please note that this does not take the place of a regular eye exam, and we cannot provide glasses based on these screenings.
