New Establishment License Application – Complete All 3 Steps
- Visit the Environmental Health Tab below and select the establishment type you are applying for.
- Complete the relevant application and forms listed and submit it by mail or email to:

Once your application and payment have been received and processed, Environmental Health Specialist, Brooke will contact you to schedule your pre-inspection.
WIC:
Taylor County Health Department no longer hosts the WIC Program. WIC Services are provided through Healthfirst: 1-800-246-5743. The goal of the Women, Infants and Children (WIC) Program is available for low to moderate income pregnant women, recently delivered women, breastfeeding women, infants, and children up to age 5 who are at nutrition risk. Fathers, grandparents & foster parents can also apply for WIC services offered in Medford, Gilman and Rib Lake.
