The full-time Case Manager-Adult Protective Services/Crisis duties include providing adult protective services [APS] investigations, crisis assessments, safety planning and decision making for placement decisions, case management services, and coordination of services for vulnerable individuals in our community. Individual will participate in on-call or after-hours consultations, provide advocacy and crisis follow-up for consumers including engaging family or caretakers in methods to solve problems, as well as, making appropriate referrals to other agencies and resources.

Applicants must possess a Bachelor’s Degree in Social Work, Human Services, Psychology, or closely related field. Prior work experience in a human services related field and certification as a state of Wisconsin social worker preferred. A valid driver’s license is required.
